I know some of you have already recieved this from Ms Mok or someone else, but if you have not, please go to the school website at nyps.moe.edu.sg and scroll down. In yellow, there is a travel declaration form that needs to be filled.
There are 2 things you need to do.
1st, you have to email Ms Yeo at yeo_ai_ho_serene@nyps.edu.sg a softcopy of the form you can find on the website attached. By 25th June, 12 nn
2nd, you need to print out a hard copy and BRING IT ON THE FIRST DAY.
Also, remember to BRING YOUR ODT!!!
